The Skilled Nursing Program

Provides transitional care to meet the needs of patients who require short-term professional care, but who would not benefit from a continued stay in an acute care setting. The focus of this program is high intensity skilled nursing and or skilled rehabilitation. The facility focuses on stroke, orthopedic, diabetic education programs, complex wound care, specialized infusion therapy, post-surgical recovery programs and cardiac rehabilitation. The patient and family become actively involved in care along with interdisciplinary team members to ensure optimal health and recovery.

We subscribe to an interdisciplinary approach to your care. This means that everyone on our treatment team works together to ensure that you receive the best care and that we maximize your recovery. Your team includes, but is not limited to:

* Nursing
* Physical Therapy
* Occupational Therapy
* Speech/Language Pathology
* Social Worker
* Physician
* Dietitian
* You and Your Family

Will My Insurance Pay?
Medicare and some insurances pay for the skilled nursing and rehabilitative services provided in our Skilled Nursing Unit. Some insurance plans will also cover your co-payment responsibility. A representative from the SNU can answer any questions you and your family may have about your benefits and coverages.
