Family Life Care Management Agency provides support services for physical, mental, emotional and spiritual well being for all ages and abilities. Sometimes things happen that challenge independent living for you or your loved ones. You may need temporary help until things run smoothly again, or simply could use a little help with the tasks of daily living. You may wish to have someone help you on a regular basis with chores that aren't so easy anymore, or you may need help every once in a while.
You may need meaningful activities to encourage friendships with others in the community or want daily telephone contact, transportation or a companion. Perhaps, you live far away from someone in your life needing a little extra -- we can be there for you. Call and let us show you how our experience and commitment can be of service to you.
Medication Supervision
Services provide a licensed nurse to set up weekly medications in a daily reminder book as ordered by the physician. Certified Nursing Assistants may give the medications and record the activity (nurses will review CNA's log to determine medications have been correctly administered correctly).
Nursing
Services prescribed by a physician consisting of either part time or intermittent nursing care provided by a registered nurse or license practical nurse within the scope of the Florida Nurse Practice Act.
Non-Residential Support Services
Individualized training activities provided to developmental disabled adults and children for the purpose of providing access to community-based activities, geared to enhance acceptable individual behaviors.
In-Home Support
Support worker provides companionship and personal care, and may assist with or perform activities of daily living and other duties necessary to maintain the recipient in supported living, from 4 - 24 hours a day assistance for persons with disabilities.
Respite Care
Service that provides supportive care and supervision to an individual when the primary caregiver is unable to perform these duties due to a planned brief absence.
Certified Nursing Assistance
Service provided by someone who has been certified and maintained quarterly trainings to perform the tasks of a home health aide in a home setting including hands on personal care. Services include, but not limited to, assisting in medication, simple procedures as an extension of therapy or nursing assistance.
Personal Care Assistance
A service for the elderly or disabled persons needing more extensive care than family or friends can provide. For some, activities of daily living, such as dressing, bathing or grooming, can be very difficult. A loved one recovering from surgery, an illness, or following hospitalization may need either temporary or long-term assistance.
Daily Telephone Contact
Services include daily telephone calls (at the same time each day) to assist in preventing the client from becoming lonely and to re-assure the safety of the client.
Transportation
For a growing number of our elderly and disabled population, the feeling of confinement is just as frustrating and depressing as their daily loneliness. Our specialized transportation service gives many of these special people the freedom to venture outside their confined environments. Many of our clients use our service to:
* Visit family and friends
* Meet medical and dental appointments
* Attend church and other social events
* Go shopping
* Enjoy a short ride through the country
In addition to providing a means of transportation, our service often helps stimulate morale and self-esteem while adding excitement to the lives of individuals as they plan activities outside their homes.
Meal Preparation
Services include preparation of meals which are nutritional, using resources available in the home. Cleaning up after preparing the meal to allow for utmost state of cleanliness and safety.
Companion/Sitter
A service providing individuals a companion to assist with regular activities like going for walks, helping with hobbies and crafts, shopping assistance, encouraging and participating in stimulating activities, and helping with correspondence to family and friends. We offer over night sitters for that extra comfort in knowing someone is available at all time. A companion may not provide hands-on personal care to a client.
Homemaker
A service providing household chore service including, but not limited to, housekeeping, meal planning and preparation, shopping assistance, and routine household activities including laundry for an elderly, handicapped, or convalescent individuals. A homemaker may not provide hands-on personal care to a client.
Chore Services
This service is to help maintain the individual's home and property in a clean, sanitary environment. These services include, but are not limited to, heavy household chores such as washing floors, windows and walls, tacking down loose rugs and tiles, replacing a broken window, or moving heavy items of furniture to make the home safer.
Hospital to Home
This service was created for people who need assistance after hospital discharge to safely arrive at home. This service includes:

2. Prepare a light meal and clean up
3. Then settle in with care

Counseling Services
When this service is needed assistance will be provided to locate a certified professional if one is not available on staff.
Share the Care
Share the Care is a program designed primarily for facilities that may need extra staffing on occasion. One call to Family Life Care is all you need; we will send a Nurse, Certified Nursing Assistant, companion or housekeeper to help. With this service you only pay for the hours of assistance you need. Most of the time when extra staff is called in, the facility must pay for a full day, often at time and a half rates. But not when you utilize our Share the Care program
The facility may have a resident who needs "just a little extra help" to be independent. You may not have the staff for one-to- one care. When you Share the Care with Family Life Care, you may be able to keep a resident with your facility rather than losing the resident to a more skilled placement.

A Gift of Caring
Treat your friend or loved one to a special day of help with those things that are difficult for them to do. Gift certificates are available by the hour or the day. They will appreciate your thoughtfulness and caring.
