Tri County Nursing Home has become a reality because of the pressing need for a first-class nursing facility in the Lower Suwannee River Valley embracing the Florida counties of Gilchrist, Dixie, and Levy. Area citizens recognized and responded to the need, forming a not-for-profit entity to develop and operate the quality healthcare center they knew would be a great asset to the community. So Tri County Nursing Home came into being as a result of there being caring people.
ROUND-THE-CLOCK ATTENTION
Our Skilled Nursing units provide roundthe- clock attention from compassionate, licensed nurses. Residents receive quality nursing care and other health-related services by trained and sensitive professionals. Our facility offers intensive rehabilitative therapy in our fully-equipped therapy departments. Our licensed therapists’ primary goal is to overcome or minimize each disability and to restore all patients to their fullest potential.
Residents receive the finest nursing care available while they recuperate to full strength in comfortable rooms. They enjoy sunny courtyards, community lounge and dining room. Friendly neighbors and a relaxed attitude make what could be a difficult period an enjoyable experience.

Our Intermediate Care unit is designed especially for those in need of less intensive nursing care. Licensed nurses will supervise and assist with medication administration, ambulation and other daily living activities to the degree appropriate.
We are licensed by the State of Florida and are certified for Medicare, Medicaid, and the Veterans Administration.

SKILLED NURSING
Our Skilled Nursing units provide round-the-clock attention from compassionate, licensed nurses. Residents receive quality nursing care and other health related services by trained and sensitive professionals.
INTERMEDIATE CARE
These important services are included in our daily room rate:
* 24-Hour Licensed Nursing Service.
* Emergency call/response system conveniently located in each room.
* Full bath with shower in every room in the nursing care units.
* Seventeen private rooms with private baths are available.
* Nutritional services supervised by a Registered Dietician.
(Including 3 meals, snacks and Therapeutic Diets)
* Housekeeping and linen services.
* Scheduled and Unscheduled recreational activities.
* Spacious, attractive patio, gazebo and community rooms for residents' use.
* Individual heating and air conditioning for personal comfort.
SPECIAL CARE UNIT
Caring for a dementia patient is a physically and emotionally exhausting job. A patient's needs may be more than can be handled at home. But there is help!
Modern care and treatment for Alzheimer and dementia related diseases is administered at Tri County Nursing Home is a distinct Special Care Unit. This unit of specially trained nurses and professionals utilizes close supervision, enhanced staffing ratios, and situation-sensitive design to replace the archaic practice of over medication or excessive use of physical restraints.
All rooms are plainly visible and accessible to the special trained staff. Here residents share a spacious, common living and dining room and have unrestricted access to an enclosed, well developed and landscaped courtyard complete with old-time gazebo.
LONG TERM CARE
For patients needing long-term care, our therapy department offers ongoing programs to assure maintenance of the highest possible level of functioning and quality of life.
We take a multi disciplinary approach to each resident's care. The rehabilitation department monitors the resident's physical ability. Dietary staff keeps abreast of changes in eating habits, swallowing problems, or significant weight changes. Activity staff note changes in social patterns and communication. Social Services observe changes in attitudes or behaviors which may be significant.
REHABILITATION SERVICES
Tri County offers professional, full-time "in-house" Physical, Occupational, and Speech/Language therapies to both our acute and long-term residents.
Physical Therapy – Ambulation, balance, restoration of muscle strength, and range of motion.
Occupational Therapy – Hand and Arm function, self care and self feeding, activities of daily living.
Speech Therapy – Speech/Language recovery, oral motor functioning, swallowing, dementia.
LONG TERM CARE
For patients needing long-term care, our program provides 24-hour licensed nursing care in conjunction with other specially trained professionals to assure maintenance of the highest level of functioning and quality of life for each resident. Our warm, friendly environment, personalized approach to care, and dedicated staff combine to ensure the delivery of optimal service to our extended stay patients and their families.
We take an integrated, multidisciplinary approach to each resident’s care. Skilled nurses monitor and respond to changes or declines in the resident’s physical condition. Dietary staff keep abreast of changes in eating habits, swallowing problems, or significant weight changes. Activity staff note changes in social patterns and communication. Working collaboratively, the team strives to create and maintain a safe, comfortable, and dignified environment for our long-term residents.
